1 Start Xephyr
2 Close it using the window manager

-> /tmp/.X$DISPLAY.lock is still there

This is not unique to Xephyr.  Xnest does the same thing.  They are using the 
default IOError handler (_XDefaultIOError) which immediately exits.  They need 
to OsCleanup()
